WHO ARE QUALIFIED?

Active, Retiree/Pensioners, and Regular Civilian Employees of:

  • Armed Forces of the Philippines (AFP)
  • Philippine National Police (PNP)
  • Department of National Defense (DND)
  • Armed Forces and Police Savings and Loan Association, Inc. (AFPSLAI)
  • Armed Forces and Police Mutual Benefit Association, Inc. (AFPMBAI)
  • Armed Forces and Police General Insurance Corporation (AFPGEN)
  • Armed Forces of the Philippines Retirement and Separation Benefits System (AFPRSBS)
  • National Police Commission (NAPOLCOM) and selected sub-agencies of the Department of the Interior and Local Government (DILG), such as Bureau of Fire Protection (BFP), Bureau of Jail Management and Penology (BJMP) and Philippine Public Safety College (PPSC)
REQUIREMENTS

Military Officer / Enlisted Personnel
  • Call to Active Duty (CAD) / Extended Tour of Active Duty (ETAD) for Officers (entry level)
  • Enlistment or Re-enlistment Order for Non-Officer (for entry level)
  • Military ID and additional one (1) valid ID, if necessary
  • Two (2) 2x2 ID photos, preferably in uniform
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
PNP Officer / Non-Officer
  • Permanent Appointment Order (for entry level)
  • Latest payslip (alternative)
  • PNP ID and one (1) additional valid ID
  • Two (2) 2x2 ID photos, preferably in uniform
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
BFP / BJMP Officer / Non-Officer
  • Bureau Order / Permanent Appointment Order
  • Latest payslip (alternative)
  • BFP / BJMP ID and one (1) additional valid ID
  • Two (2) 2x2 ID photos, preferably in uniform
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
AFP / PNP / BFP / BJMP Retiree - Pensioner
  • Retirement Order (if not yet receiving pension)
  • Certification of Pension or copy of latest pension check or latest Monthly Verification Slip provided original copy is presented
  • Retiree’s/Pensioner’s ID and one (1) additional valid ID
  • Two (2) 2x2 ID photos
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
AFP / PNP / BFP / BJMP - Pensioner (Widow/er) / Legal Beneficiary / Survivor
  • Certification of Pension or Certification/Declaration of Legal Beneficiary from OTAG/JAGO or its equivalent (if not yet receiving pension)
  • LAtest Monthly Verification Slip / STI3
  • Pensioner’s ID and two (2) additional valid IDs
  • Two (2) 2x2 ID photos
  • CRS#5 Advisory on Marriages
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
Inactive / Reserve Officer
  • Statement of Service Records from OTAG - must be Called to Duty (CAD) for 1 term or 3 continuous years
  • Two (2) valid IDs
  • Two (2) 2x2 ID photos
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
Civilian Employees (AFPSLAI-accredited institutions)
  • Appointment Order
  • Latest Payslip (alternative)
  • Company ID and one (1) additional valid ID
  • Two (2) 2x2 ID photos, preferably in uniform
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account
Resigned from Service / Honorably Discharged
  • Discharged Order, such as:
    • Special Order - for AFP Enlisted
    • General Order - for AFP Officers
    • Statement of Service - for PNP Personnel
  • Two (2) valid IDs
  • Two (2) 2x2 ID photos
  • ₱1,000 initial deposit for Capital Contribution Account
  • ₱100 initial deposit for Savings Deposit Account

Application at the Branch Offices

  1. Member proceeds to Membership Section.
  2. Membership Staff screens eligibility of the member based on the prevailing By-Laws and Association policies.
  3. Member fills out application forms for Membership, ATM ID Card and Mobile App, and submits documentary requirements.
  4. Membership Staff verifies the requirements and encodes data in the system.
  5. Member fills out Specimen Signature Card (SSC), presents valid IDs and submits 2x2 pictures.
  6. Membership Staff issues CCA (and SDA) passbooks.
  7. Member proceeds to the Teller Section to make an initial deposit of Php 1,000 for Capital Contribution Account and Php 100 for Savings Deposit Account. Also, member pays Php 100 for ATM ID Card fee.
  8. Member proceeds again to the Membership Section to present the Official Receipt (OR) and to process the ATM ID Card (member data capturing and printing of card).
Online Application

  1. Member visits the AFPSLAI website (www.afpslai.com.ph).
  2. Member clicks the e-Application Form (Membership Application).
  3. Member fills the required fields and waits for the reference number.
  4. Member proceeds to the Membership Section to show the reference number and submit the original requirements.
  5. Membership Staff verifies requirements and encodes data in the system.
  6. Member fills out application forms for Membership, ATM ID Card and Mobile App.
  7. Member fills out Specimen Signature Card (SSC), presents valid IDs and submits 2x2 pictures.
  8. Membership Staff issues CCA (and SDA) passbooks.

FAQs

Q: Is there a membership fee for qualified applicants?
A: We do not collect any membership fees, but basic requirement includes a Php 1,000 nonwithdrawable deposit to open the Capital Contribution Account (CCA). An AFPSLAI ID shall likewise be provided to new members free of charge.

Q: In the absence of AFP/PNP ID, what can I present to support my membership application?
A: In lieu of AFP/PNP ID, a certification (from the Administration Office, OTAG or its equivalent entity) that the specific ID is being processed may be accepted. Said certification must be duly signed by the designated/authorized Finance Office Head or his/her authorized representative

Q: Can I be a member at once?
A: Yes, provided all membership requirements are complied with.

Q: I was a member before. Can I be a member again?
A: Yes, AFPSLAI allows re-admission of members as long as you are still qualified under our existing Membership By-laws. Requirements are the same as the requirements for new members. Member should also submit a re-admission request letter together with the requirements. The format for the said letter is available in the Membership Section.
